Our Registered NurseCase Managers are at the very heart of the compassionate care we provide. This role offers you the ability to use your nursing and case management skills in a new way in a post-acute care environment. This is a great opportunity for Registered Nurses with at least one year of Home Health or Hospice experience to move into case management. Our Registered NurseCase Managers develop, implement and assess care plans for individual patients using the principles of community health nursing. This includes performing skilled assessments of patients; formulating written care plans and ensuring appropriate documentation is completed. You will also provide skilled nursing care and instruction to patients, as well as supervising a group of caregivers regarding patient care and health status. Additionally, youll interact with physicians, solve problems and oversee patient care, all with the goal of providing our clients with the best care available. To qualify, you must have a current CA RN license and at least one year of full time experience as a Registered Nurse, ideally in a med/surg acute-care setting. Prior home care experience is desirable. Registered Nurses with at least 2 years SNF background seeking to move into case management will be considered.
